A lisp occurs when airflow or tongue placement changes the expected sound of “s,” “z,” “j”, “sh,” or “ch.” While a lisp itself is a speech pattern, a tongue tie is a structural and functional restriction; when those two overlap, clarity takes a hit.

Let’s break that down now:
- Interdental lisp – Perhaps the most well-known and recognizable type is the interdental lisp. Here we see that the tongue pushes out between the teeth, exposing the tongue tip when the sound is produced. Many times, the production may “sound” ok to the listener, and other times there may be excess air, creating a hissing quality to the S production.
- Dentalized lisp – In a Dentalized lisp, the tongue touches/pushes into the front teeth, dulling the sound. At times, the sound can seem forced and have a ‘ts” quality to the production.
- Lateral lisp – A lateral lisp occurs when the air escapes over the sides of the tongue, creating a “wet” or “slushy” sound into the cheeks. This lisp is a bit more distracting to the listener, as it adds a noticeable distortion quality to the speaker’s words due to excess air escaping.
All of these depend on precise tongue mobility and coordination—two abilities that tethered oral tissues compromise.
Correct Production of Sibilants
Sibilants are a group of sounds that require airflow to be produced. This group includes S, Z, SH, CH, and J. To better understand the distortion, let’s first examine the correct production in order of speech sound development.
Starting with S:
Developmentally, this is the first sibilant sound we can produce as a three-year-old.
To correctly produce;
- Seal teeth
- Retract and side-spread the posterior section of the tongue
- Posterior tongue then laterally braces against the molar gums
- Coordinate airflow to flow over the tongue
- Slight groove in tip as it aims the air stream out through the front teeth
- Lips are relaxed and retracted in a smile. For some vowels in which the lips are rounded (soap) there can be a slight purse to the lips
- Cheeks squeeze inward, creating an oral cavity seal along the teeth and gums to help direct airflow down the runway of the tongue.
To Produce Z:
- Same as above with the addition of the “buzz”.
- To Buzz, we turn on our voice and add noise into the air-stream.
SH is Next:
- Now we use our cheeks and lips more
- Rounded lips to shape the SH sound
- Squeeze and push forward from the cheeks to assist in the rounding of the lips
- While lateral bracing, as in the S, continues, we also have a slight dip in the mid tongue occur
- Continual airflow, as in S, is utilized
CH Steps up:
- Same shaping as in SH
- Short burst of air
- Increase tension in the tongue to push slightly against the front teeth (similar to T) to reduce the speed of the airflow.
Finally J:
- We are back to the “buzz”
- J is the CH sound with vibration
- Same shaping as the CH, but now you add in voicing to add noise into the short burst air-stream.
The key motor plan in all these sounds is the side-spread and lateral bracing of the posterior tongue. If the tongue is anchored, as it would be with a tongue tie, then it can not elevate and side spread to achieve lateral bracing.

A tongue tie occurs when the thin membrane beneath the tongue (the lingual frenulum) is too short, thick, or tight to allow a full range of motion.
In childhood, this can affect breastfeeding, swallowing, and early speech. But in adults, a tongue tie influences oral rest posture, airway function, facial balance, chewing, swallowing, and articulation long after those early years.
Many adults adapt by overusing their jaw, cheeks, or lips to speak. Over time, that compensation shows up as tension, fatigue, or speech distortion—including lisps.
